Functional Medicine
This is not your typical doctor’s visit. The Functional Medicine New Patient Appointment is designed to cover everything from a comprehensive health intake, recommended innovative lab testing, lifestyle modifications, and nutritional support. All appointments are currently via telehealth and available to all Massachusetts residents.
Functional Medicine is a personalized, systems-oriented approach that empowers patients and practitioners to achieve the highest expression of health by working in collaboration to address the underlying causes of disease.
Your initial functional medicine consult.
Your initial telehealth appointment will include a 60-minute consultation with Dr. Zubiel. During your consultation, she will review your personal and family health history, your goals, and your current ailments.
Nutritional therapy and lifestyle modifications will be discussed and are integral components of your wellness plan. Through targeted lab testing (optional), we are able to assess how each body system is functioning and design an more specific program to rebalance your body.
Nutritional supplements are often recommended initially, and we will help you select and find the highest quality products.
What do I need to do to get started?
You may book your initial appointment online.
Comprehensive online intake forms must be completed and submitted 48 hours prior to your first appointment. This allows for adequate review of your health history by the practitioner prior to your initial appointment. We reserve the right to cancel your appointment if forms are not submitted in time. Please send along any lab results that have been completed within the past 6 months.
Our intake forms are quite lengthy in order to ensure quality care provided by our staff. The more thorough and accurate the information provided, the better your health outcomes. Essex Integrative Health, LLC keeps your information private pursuant to the terms of our Privacy Policy.
How are follow-up appointments scheduled?
Dr. Zubiel will provide a follow-up schedule during your initial consult that suites your specific needs, but you may schedule an appointment at any time for additional support.
If applicable, once all lab results have been received we will notify you to schedule your first follow-up consultation to review the results. Together, we’ll create an individualized health program, which will include any additional dietary modifications, nutritional supplements, and exercise, lifestyle and stress management recommendations. We’ll prioritize your protocol and establish a relationship for on-going support.
Subsequent telehealth consultations are scheduled to monitor your progress and to modify your program accordingly. These sessions provide time to review your personalized plan and discuss next steps on your journey.
What is the fee for a functional medicine appointment?
The fees vary based on appointment type and time.
Initial appointment: $275 (60 minutes)
Follow-up appointments: $135 (30 minutes)
Follow-up appointments with lab review: $195 (45 minutes)
Due to schedule availability, full payment is required at the time of booking and New Patient Appointments are non-refundable. All major credit cards, including HSA and FSA cards, are accepted.
We do not accept or participate with any health insurance companies. Upon request, we will provide you with a superbill (contains diagnosis and CPT codes for services performed) to submit to your health insurance for possible out-of-network reimbursement.

Online Appointments (Telehealth) System Requirements
Online appointments are currently supported and best experienced on a computer or laptop using Google Chrome. Android devices work best with Chrome. If you’re on an iOS device (iPhone, iPad) you will need the Jane Online Appointments app which is available on the app store.
Check your internet connection at fast.com. You will need a result of 15Mbps or higher. If you’re seeing a number measured in Kbps, that’s not good news. If your internet connection isn’t great, you will have the option to shut off the video and do an audio-only session.
