Do you take insurance?
No. Essex Integrative Health, LLC is not in-network with any insurance plans, including Medicare. Since we do not take insurance, we cannot bill insurance for you.
Upon request, we will provide you with a superbill (including ICD-10 codes for diagnosis and CPT codes for services performed) to submit to your insurance for possible out-of-network reimbursement. We recommend that you call the member services number on the back of your insurance card and inquire about your coverage for services performed by an out-of-network chiropractor.
What type of payment is accepted?
Payments are made through your Jane account that’s created when you schedule your first appointment. Visa, Mastercard, American Express and HSA/FSA cards are accepted.
How are patients billed?
Due to limited schedule availability, full payment is required at the time of booking. All major credit cards, including HSA and FSA cards, are accepted.
How much do lab tests cost?
Depending on the lab test, the cost is typically $100-$450 per lab kit. Most labs are run through Rupa Health. Payment is due at time of ordering the lab kit. Rupa Health provides the option to pay using Credit, Debit, HSA, FSA, or a 3-Month Payment Plan.
What is your cancellation policy?
If for any reason you cannot make your appointment, please let us know within 24 hours so we can reschedule accordingly. A cancellation fee equal to the fee for the service that was scheduled and missed is applied if the appointment is cancelled with less than 24 hours notice. Please review our Terms of Service for more information.
Can I use my FSA/HSA for consultation fees or other costs?
